Investigating the role, experiences and quality of life of carers in SA

This survey will investigate the role, experiences and quality of life experienced by carers. Your input will provide an important insight into the impact (both positive and negative) that carers experience physically, emotionally, socially, professionally and financially.

All carers are welcome to complete the survey regardless of your age, or the age of the person you are caring for. Participation in this survey is entirely voluntary, confidential and anonymous. Undertaken through the Department of Rehabilitation and Aged Care at Flinders University.

The survey can be completed at www.flinders.edu.au/medicine/sites/raec/carers-survey.cfm.

OR on paper: contact Associate Professor Julie Ratcliffe or Ms Leah Couzner 8275 1103, by email at julie.ratcliffe@health.sa.gov.au or leah.couzner@health.sa.gov.au.
